Graduate Students Scheduled for a Clinical Rotation/Clinical Fieldwork
Step 1: Before you arrive, please carefully read and review our Children’s Therapy TEAM Fieldwork Education Guide.
Step 2: At least 2 weeks before you arrive, please complete a Student Facesheet.
Step 3: Upon arrival at our clinic, please be prepared to present your student ID and complete our HIPAA consent form.
Students Scheduled for an Observation
Step 1: Before you arrive to observe read our Welcome Pack for Student Observers.
Step 2: Before you arrive to observe also submit an online Observation Verification Form (OT student observers only). This should be completed after you have confirmed your observation schedule with your supervising therapist.
Step 3: Upon arrival at our clinic, please be prepared to present your student ID and complete our HIPAA consent form.
